Voluntary Group Benefits

During Open Enrollment, employees have the option to participate in voluntary group benefit plans.  All premiums may be paid via payroll deductions.  Unless otherwise noted, application for these benefits may only be made during open enrollment.  Additional plan information is available from the links below and plan representatives and enrollment material will be available at the open enrollment meeting.

Voluntary Plans  

Group Benefits from The Harford:   

Current employees - Term Life insurance is available in $10,000 increments for a maximum of 5 times your annual salary, not to exceed $300,000. Your spouse/domestic partner is eligible for coverage in $5,000 increments to a maximum of 50% of the employee’s purchase amount, not to exceed $150,000.  Applications will be submitted to Mutual of Omaha medical underwriting.
Newly hired employees - *Guarantee issue: Employee $150,000; spouse or domestic partner $75,000; dependent children $10,000.

*Application for guarantee issue must be completed within 31 days of hire date.

For $19.50 a month, you get legal assistance for some of the most frequently needed personal legal matters — with no waiting periods, no deductibles and no claim forms, when using a network attorney for a covered matter. MetLife Legal Plan information

 UNUM Life Insurance Company of America

UNUM LTC coverage includes expenses for nursing home facilities and home health care.  You may choose from coverage levels beginning at $1,000 per month, up to $6,000 per month, for nursing home care.  Eligibility is extended to you, your spouse, parents, parents in-law, grandparents and grandparents in-law.  The monthly premium varies based on age and the level of coverage chosen.  Applications will be submitted to UNUM medical underwriting.  Newly hired employees - *Guarantee issue: 2 years/$1,000 per month.  *Application for guarantee issue must be completed within 31 days of hire date.  Contact the Human Resources Office in enrollment information..

FBC Deferred Compensation Program

PLEASE NOTE:  In order to provide better service to our member districts and charters, the Deferred Compensation Program has implemented a centralized email and phone service for all general inquires. This will allow us to track and ensure a prompt response, with consistent information. This will also save participants time by helping them route to the appropriate third-party administrator (TPA) or vendor. Moreover, this information will remain consistent when staff changes, so fewer updates and changes will be needed for forms and websites.
